Advanced Practice Provider (NP or PA) – Internal Medicine / Family Medicine

Outpatient Primary Care Practice

Position Overview

We are seeking a dedicated Advanced Practice Provider (NP or PA) to join our outpatient Internal Medicine / Family Medicine team. This role focuses on building long-term patient relationships while providing comprehensive primary care services.

Key Responsibilities

• Provide preventive care, annual physicals, and wellness visits

• Diagnose and manage acute and chronic conditions

• Manage medication therapies and care plans

• Order and interpret labs and diagnostic testing

• Educate patients on disease management and preventive health

• Coordinate referrals and specialty care

• Maintain accurate documentation in the EMR

• Collaborate with physicians and care team members

Qualifications

• Licensed NP or PA in good standing

• Primary care experience preferred

• Strong patient communication and relationship-building skills

• Commitment to high-quality, patient-centered care

Schedule & Environment

• Outpatient only

• Monday–Friday schedule (no nights or weekends)

• No inpatient responsibilities
