Imagine if you will….You visit your small panel of patients in the comfort of their own homes. You spend all the time needed to ensure your patients’ health issues are addressed. You create a friendly and trusting relationship with each patient and his or her family. You have the support of a collaborative, multidisciplinary clinical team who works in the field with you. And you do it all at no added cost to the patient.

Primary Responsibilities:

Under the supervision of a medical doctor:

  • Available  to conduct follow-up visits, including past medical history, review of symptoms, non-invasive physical examination and medication review
  • Identify diagnoses to be used in care management
  • Communicate assessment findings that will be used to inform the PCP of potential gaps in care
  • Educate patients and caregivers on disease process, medication and compliance
  • Comply with all HIPAA regulations
  • Ideally available to travel to different counties in Dallas/Fort Worth as needed. 

Required Qualifications and Skills:

  • Active, unrestricted license to practice as PA in the State of Texas
  • Physician Assistant Board certification 
  • Ability to obtain DEA licensure/prescriptive authority in the state of Texas 
  • Reliable transportation in order to travel to patient homes within a designated area
  • Excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
  • Knowledge of eClinicalWorks (preferred)
  • Bilingual (English/Spanish) a plus
